Wise Recovery Transitions
A structured, compassionate approach to the transition from hospital to home
This initiative supports recovery continuity across the transition from hospital to home and is currently focused on books for patients and families. Over time, it will expand into a comprehensive, recovery-focused program that spans the full hospital-to-home continuum—beginning in the hospital, supporting discharge planning, guiding patients and families after discharge, and extending into outpatient care.
As it evolves, this work will also grow to support multiple participants in the recovery process, including patients, families, mental health providers, and learners, through shared tools that strengthen understanding, communication, and follow-through across care settings.
The first book in this project, The Next Day: Guidance for Recovery After a Mental Health Hospital Stay, is currently in development.
